Sumario: | On 28 August 1997 an unexplained erroneous modification of a software interlock in the timing system of the SPS beam dump kicker has led to wrong settings of the beam dumping system. Continuous operation of the SPS under these conditions has led to a high radiation level at ring position 3:07 and finally to a leak in the vacuum chamber of a dipole magnet. This note identifies the causes of this accident and proposes solutions in order to avoid the similar problems in the future |
