The disease reached spaces where human, history and social confluence-imposed scenarios of health and global catastrophe. The baseline risk pattern in Mexico assumed evident characteristics of a vulnerable population, a disease burden with metabolic risk factors and chronic diseases in preponderant sites, as well as unresolved lifestyles and communicable diseases; in a scenario of deteriorated social conditions and a health sector, which facilitated the damage in a damaged space of origin. Objective, analyze the comprehensive bases of the health-disease-care process, linked to COVID-19 and show the potential scenario of collective vulnerability for the recovery period. METHOD. Analytical cross-sectional epidemiological design with critical orientation and theoretical perspective in collective health. RESULTS. Mexico has maintained the highest fatality figures in the world, where 1 in 10 patients die and increase in areas of inaccessibility of health services, with systematically high rates of positivity to the disease and risk of dying from comorbidities and various conditions and modifiable. CONCLUSIONS. The space for the care of the disease and the necessary concurrent recovery of COVID-19 is developed and expected in a scenario with a complex disease burden, deteriorating social conditions and, therefore, a company that requires a reorientation of State policy attending to criteria of primary health care, therefore, beyond the responsibility of health services.

El desarrollo gradual y a largo plazo de la pandemia atribuida al agente SARS-CoV-2 ha incrementado la complejidad en su expresión y el efecto negativo que ejerce sobre el ámbito colectivo. La enfermedad llegó a espacios donde la confluencia humana, la historia y lo social, impusieron escenarios de catástrofe sanitaria y global. El patrón basal de riesgo en México suponía características de una población vulnerable evidentes, una carga de enfermedad con factores de riesgo metabólico y enfermedades crónicas en sitios preponderantes, además de modos de vida y enfermedades transmisibles no resueltas; en un escenario de condiciones sociales y un sector salud deteriorados, lo cual facilitó el daño en un espacio vulnerado de origen. Objetivo, analizar las bases comprensivas del proceso salud-enfermedad-atención, vinculado a la COVID-19 y mostrar el potencial escenario de vulnerabilidad colectiva para el periodo de recuperación. MÉTODO. Diseño epidemiológico transversal analítico con orientación crítica y perspectiva teórica en salud colectiva. RESULTADOS. México ha mantenido las cifras de letalidad más elevadas en el mundo, donde 1 de cada 10 enfermos mueren e incrementa en espacios de inaccesibilidad de los servicios de salud, con índices de positividad a la enfermedad sistemáticamente elevados y riesgo de morir por comorbilidades y condiciones diversas y modificables. CONCLUSIONES. El espacio de atención de la enfermedad y la recuperación necesaria concurrente de la COVID-19 se desarrolla y espera en un escenario con una carga de enfermedad compleja, condiciones sociales en deterioro y, por tanto, una empresa que obliga a reorientar la política de Estado atendiendo a criterios de atención primaria a la salud, por ende, más allá de la responsabilidad de los servicios de salud.
