description Imidacloprid is a neonicotinoid insecticide highly specific to nicotinic acetylcholine receptors in insects and other invertebrates. Nicotinic receptors in mammalian species have a low affinity to neonicotinoids. However, cross-reactivity with mammalian nicotinic receptors is a major concern especially due to the propensity of this commonly used agent to persist in environmental water sources for an extended period of time. Here, we present a case report of a patient who presented to the emergency department with features suggestive of neuromuscular junction dysfunction, following exposure to imidacloprid.
