description Focus Area: Supporting Behavioral Change As the paradigm of healthcare financing and the reality of aging adults with multiple chronic conditions confront the healthcare work force, the role of members of the healthcare team who can assist in guiding patients in behavior change from their current state to a desired state is essential. Integrative health coaches are uniquely trained and qualified to provide this critical link between medical recommendations and patient engagement and activation. Duke Integrative Medicine developed a model of integrative health coaching professional education and launched the training in 2008. Integrative health coaching through Duke Integrative Medicine is based on personalized health planning using a “wheel of health” exploration with a patient using a 9-step patient engagement process. This session will allow attendees to experience and learn the key components of integrative health coaching and the training lessons learned within the Duke model. Application and understanding of these skills is valuable for any clinician, corporate or health HR professional and health insurance executives.
