KEY CLINICAL MESSAGE: The 3D-computed tomography (CT) image helped to make the operation strategy for the resection of huge thymoma obstruction large mediastinal vessels. Even in the rare situation, preoperatively enhanced 3D-CT image is useful for a safe operation.
