description We report a case of a 45-year-old male with a syringopleural shunt who developed intracranial hypotension. The patient presented with 2 weeks history of worsening headache and back pain, on a background of having had a syringopleural shunt inserted for a thoracic posttraumatic syrinx. Computerized tomography imaging of the brain revealed bilateral subdural fluid collections. Magnetic resonance imaging appearances of spinal and intracranial pachymeningeal enhancement confirmed intracranial hypotension. We present a rare case of intracranial hypotension secondary to syringopleural shunting in a patient with posttraumatic syringomyelia.
