description Presented is the first case of acute immune reconstitution inflammatory syndrome (IRIS)-associated cryptococcal meningoencephalitis in a patient on natalizumab for multiple sclerosis. We propose several mechanisms that explain the acuity of IRIS in this specific patient population and suggest possible therapies.
