description IN BRIEF This pilot program evaluates the impact of telephone versus mixed modalities of care on A1C. A retrospective chart review was conducted to evaluate mean baseline and follow-up A1C values for all patients who received telephone care, video-conferencing, or in-person clinic appointments with certified diabetes educators at a single, rural U.S. Department of Veterans Affairs clinic. The results of this evaluation showed that glycemic control was improved both in patients who received diabetes management through telephone care alone and in those who received mixed modalities of care.
