Sumario:Severe aortic stenosis (AS) is considered a contraindication to the use of mechanical circulatory support (MCS) devices, including the Impella heart pump (Abiomed, Aachen, Germany). We describe a case in which a 72-year-old female with severe AS and peripheral vascular disease (PVD) presented with retractable ischemia in the setting of a non-ST elevation myocardial infarction (NSTEMI). Using a coordinated multidisciplinary approach, our case is the first to combine iliac angioplasty, balloon aortic valvuloplasty (BAV), and the insertion of an Impella CP device in the setting of severe AS to facilitate successful coronary artery revascularization in a non-surgical patient.
