An endoscopic ultrasound–guided esophageal approach for mediastinal pseudocysts has not been reported commonly in the literature. We report a rare case of a large mediastinal pseudocyst that was drained through the distal esophagus with eventual complete resolution of symptoms and pseudocyst.
