We here report the case of a 70-year old patient presenting with urogenital prolapse Stage III. Total double-approach hysterectomy without adnexal conservation was performed: vaginal approach complemented by laparotomy showing uterine inversion with bilateral adnexa pulled into the inversion. Abdominal approach allowed good surgical exposure, exclusion of gastrointestinal or urinary contents drawn into the inversion before hysterectomy. Although rare and difficult to diagnose, acute non-puerperal uterine inversion is a medical-surgical emergency.
