description We report the first case of spontaneous infarction occurring in pleomorphic adenoma located in infratemporal fossa mimicking parapharyngeal abscess, including clinical presentation, workups, and surgical approach. The final diagnosis was confirmed on a histological basis. The combination of CT scan and MRI might be helpful in distinguishing neoplasia from abscess collection in this area. Surgical intervention is the treatment of choice.
