description Theophylline toxicity has become rare in the 21st century, as the drug has fallen out of favor due to serious life-threatening adverse events, narrow therapeutic window and readily available therapeutic alternatives. The wide array of clinical symptoms related to theophylline toxicity makes this diagnosis challenging for the treating physician. We report a case of an elderly gentleman who presented with respiratory failure and seizures due to severe theophylline toxicity.
