description A 42-year-old male patient presented with recurrent inferior ST-segment elevation myocardial infarction with minimal atherosclerotic disease on intracoronary imaging. Transesophageal echocardiogram and computed tomography aortogram revealed the underlying cause to be a mobile aortic thrombus in the right coronary cusp, prolapsing into and out of the right coronary ostium.
