Sumario:Giant parathyroid adenoma is a rare type of parathyroid adenoma defined as that weighing >3.5 grams. Although it presents with primary hyperparathyroidism, the clinical and laboratory findings are more profound. Bone changes are common in long standing hyperparathyroidism but presentation with clubbing is rare. Here, we report a case of 60-year male with history of finger deformities and swelling in neck. On evaluation he had hypercalcemia with high PTH levels. He underwent parathyroidectomy for the giant parathyroid adenoma but had eventful postoperative period with prolonged hypocalcemia. This emphasizes the importance of timely management of hyperparathyroidism to prevent untoward consequences.
