description | BACKGROUND-: Akathisia is proven in various studies that amisulpride augmentation in patients partially responsive to clozapine is an effective strategy. The effectiveness of this strategy may be attributable to complementary receptor profile of the amisulpride and clozapine. In view of this property, we tried the reverse of this augmentation strategy, i.e. augmentation of amisulpride with clozapine but it proved to be a tragedy in our case. AIM-: AUGMENTING AMISULPRIDE WITH CLOZAPINE LEADING TO AKATHISIA: A CASE REPORT METHODS: A 32 year old female patient with a 3 year history of schizophrenia and was partially controlled on olanzapine 20 mg per day from last 1 year presented in psychiatry OPD with complaints of weight gain. In view of this, patient was switched to aripiprazole 20 mg per day. Intervention was done by switching the drug and patient was monitored on follow RESULTS: it is proven in various studies that amisulpride augmentation in patients partially responsive to clozapine is an effective strategy. The effectiveness of this strategy may be attributable to complementary receptor profile of the amisulpride and clozapine. In view of this property, we tried the reverse of this augmentation strategy, i.e. augmentation of amisulpride with clozapine but it proved to be a tragedy in our case. CONCLUSIONS: From case report, it can be suggested that we should be careful when augmenting amisulpride with clozapine.Therapeutic drug monitoring of amisulpride may also be suggested while augmenting amisulpride with clozapine to check for any changes in plasma levels of amisulpride so that appropriate action can be taken before any tragedy happens. |
