description Coarctation of the aorta (CoA) is a congenital heart disease found in a newborn with an incidence of 6%. It presents a significant clinical challenge in neonates posted for major surgeries like tracheoesophageal fistula (TEF) repair. We report the case of anesthetic management of a 2-day-old infant with CoA and duct-dependent circulation posted for TEF repair. We describe how physiology affects its perioperative management and the role of maintaining balance in peripheral vascular resistance and systemic vascular resistance to maintain ductal flow.
